INSTRUCTIONS

According to the 2016 Minnesota Statutes, the school leadership team at each school site must develop a Full-Service Community School (FSCS) Plan detailing the steps the school leadership team will take to implement FSCS programs. The FSCS Plan must meet the requirements described in the 2016 Minnesota Statutes 124D.231 subdivision 2 sections g and h. You may use this form, which addresses all of the required elements, or you may create your own FSCS Plan template that includes all of the required elements.

  1. School Leadership Team

  1. Please describe how the school will ensure the consistent operation of the school leadership team and ongoing monitoring of the development and implementation of the full-service community school operations and programming at the school site.

The community school at Denfeld is supported by a site leadership team that meets monthly to maintain the focus and monitoring of the development and implementation of the FSCS. Denfeld also maintains a continuous improvement team dedicated to connecting the development of the FSCS with the data driven progress of school improvement.   

  1. Please describe the plan for school leadership team development.

Denfeld dedicates a portion of its budget annually to professional development, and commits to provide these opportunities in tandem with the needs identified by the FSCS site team and coordinator, in order to best support the development of the FSCS.

  1. Please describe how the school will inform parents and community members of the school leadership team meetings, agendas, meeting minutes, and annual reports.

Information on the FSCS site team meetings will be made available in family newsletters sent out monthly. Details including meeting agendas, minutes, and annual reports will be accessible via the FSCS area on the Denfeld website.

  1. Community Stakeholders

  1. Please describe how the leadership team will ensure and document meaningful and sustained collaboration between the school and community stakeholders, including local governmental units, civic engagement organizations, businesses, and social service providers.

Many local stakeholders, both nonprofit organizations, parents, students, and community members, are involved in the Denfeld Site Leadership Team, which meets monthly to advise, guide and support the FSCS at Denfeld.

The Duluth Community School Collaborative (DCSC) creates and keeps on file a memorandum of understanding with each partner organization, business, provider, and governmental unit with which they collaborate, and have contracts with regular community consultants. In addition, records are kept by the Denfeld site coordinator of informal, or short term partnerships, and community focus groups/feedback.

  1. Please describe the plan for establishing and maintaining partnerships with institutions, such as universities, hospitals, museums, or not-for-profit community organizations to further the development and implementation of community school programming.

Denfeld FSCS site coordinator is already building partnerships with the University of Minnesota, Duluth medical school, School of Social Work, and student groups, as well as student groups and Dignitas classes at College of St Scholastica, and the multicultural education program at University of Wisconsin, Superior. Plans to deepen these relationships through formal partnerships for student service learning and internships, as well as program collaboration are being developed currently. Our district has relationships with technical colleges and businesses through its vocational program coordinator, who is also working with the Denfeld site coordinator to build applicable OST programming. The Denfeld site coordinator is a member of DYAC, Duluth Youth Agency Collaborative which meets monthly and includes multiple youth serving agencies, to stay connected to those nonprofits. Finally, the Denfeld site is a member of the Duluth Community School Collaborative’s Together for Health initiative, which includes a multitude of local healthcare corporations and stakeholders with the goal of building healthy communities, and these meetings happen at least twice a month.

Our primary FSCS program is the Denfeld After School Happenings (DASH) Program, which includes many of the above programming types within a structured OST partnership framework.

  • The DASH program is open to all Denfeld students, with a focus on students in need of academic support and those not otherwise engaged by team sports or other after school clubs/activities at the school.

  • DASH will address needs of:

    • healthy food & basic needs access by providing a free afternoon hot meal and extended student food shelf/clothing closet hours

    • opportunity gap by providing free after school programming

    • academic support by providing after school tutoring

    • college and career readiness by providing a college student mentoring group with elements of service learning, leadership, and career exploration

  • DASH partners with College of St Scholastica, University of Minnesota Duluth, University of Wisconsin Superior, Lake Superior College

  • DASH meets M-Th from 3:30-5:30 on days school is in session.

An additional program provided by the FSCS is the Family Leadership Program

  • Family Leadership Program targets any and all family members of students at Denfeld who are interested

  • Family Leadership Program addresses the needs of

    • Authentic family engagement & leadership by providing  family leadership events throughout the school year, and parent teacher home visits

  • Partners include the Parent Teacher Home Visit Project. Additional partners are being identified.

  • Family Leadership Program events will occur at least once every quarter and parent teacher home visits will begin in 2018-19 school year.

Lastly, the FSCS provides a Health Realization Program

  • Health Realization Program targets all students at Denfeld, and includes teachers, staff, and community members

  • Health Realization Program addresses the needs of

    • Mental health access by providing in school and after school access to mental health practitioners and mindfulness practices

    • Dental care access by providing free at-school dental care for students

  • Partners include Generations Health Care Initiatives and Operation Grace

  • Health Realization Program activities occur at different times throughout the year during the school day.  


  1. Each of the FSCS programs offered must include at least one goal that is Measurable and Aligned to the District’s World’s Best Workforce goals.  Please provide at least one goal for each of the FSCS programs that you will offer.

DASH Program

  • At least 50% of DASH Program participants will attend at least one college mentorship and career pathways opportunity outside of the school day.

Family Leadership Program

  • Family Leadership Program will provide families at least 4 opportunities throughout the school year to participate in engagement events aimed at increasing graduation rates.

Health Realization Program

  • Health Realization Program will provide at least 4 opportunities each year for students to participate in holistic and physical health services to address barriers to achievement by all groups of students, assisting in closing the achievement gap.

The program coordinator will utilize the equity tool developed by the ISD 709 office of educational equity in major decisions about programming to assist in catching any areas of concern.

Attendance records will be kept on paper and transferred to a digital format each quarter for record keeping.

  1. Measuring Impact

  1. Please describe the plan for maintenance of measurable data showing annual participation and the impact of programming on the participating children and adults.

All students who participate in the after school program will be required to complete a registration form, enrolling them in the program. Following which they will be required to sign in on arrival to the program. This data will be translated to a digital format each quarter where annual data can be pulled. Parent engagement will also be tracked digitally through event sign-in’s and rsvps.
